Ulthera SMAS Lifting After 40 and 60: Who the Treatment Is Suitable For and When a Facelift Is Needed

Ulthera System SMAS lifting may be considered both after the age of 40 and after 60, but the decision is not based on chronological age. Microfocused ultrasound is used for a moderate reduction in firmness and mild to moderate tissue descent. When there is marked excess skin, deep folds, or a significant change in facial contour, a device-based treatment cannot replace a surgical facelift. A realistic treatment option can be determined only after an in-person assessment of the face and neck.

How Ulthera System SMAS Lifting Works

Ultherapy is a non-invasive treatment based on microfocused ultrasound. Energy is delivered to a selected depth, creating small zones of controlled heating within the tissues. This causes contraction of some existing collagen fibres and initiates subsequent collagen remodelling. The skin is not incised or resurfaced, so prolonged wound healing is not required.

A defining feature of the Ulthera System is real-time ultrasound imaging. The physician can see the tissue layers and select the treatment depth according to the patient’s individual anatomy. At Medest, transducers for depths of 1.5, 3.0, and 4.5 mm are used; DeepSEE technology allows the physician to verify tissue positioning before energy is delivered.

Changes develop gradually because collagen remodelling takes time, so the outcome is assessed after several months. The degree of improvement depends on the initial condition of the tissues and the individual rate of ageing.

What Changes After the Age of 40

At around 40, common concerns include a less defined jawline, early descent of the cheeks, reduced density of the neck skin, and a small fold beneath the chin. If excess skin remains moderate and tissue volume is preserved, ultrasound SMAS lifting may provide natural-looking firmness and a more defined contour without changing facial features.

However, the same age does not mean the same indications. In one patient, the primary concern may be skin quality; in another, loss of volume in the midface; and in a third, excess subcutaneous fat. Ulthera does not restore lost volume, remove localised fat, or correct active facial movement. The physician may therefore need to combine methods or select a different treatment.

Is Ulthera Suitable After the Age of 60?

Being 60 is not, in itself, an automatic limitation. More important factors include the degree of ptosis, skin thickness and quality, the amount of subcutaneous tissue, bone support, chronic medical conditions, and the patient’s expectations. With moderate laxity, a device-based treatment may improve skin density and contour. If there is a large amount of excess tissue, the result will be limited even when the procedure is performed correctly.

What the physician assessesWhen Ulthera may be suitableWhen a plastic surgery consultation is needed
Excess skinModerate, without heavy overhanging foldsMarked excess skin of the face or neck
Tissue positionMild or moderate ptosisSignificant descent of the soft tissues
ExpectationsGradual, natural-looking firmingA noticeable anatomical repositioning is required
Readiness for recoveryMinimal interruption to daily life is importantThe patient is prepared for surgery and rehabilitation

Device-based and surgical lifting are not two versions of the same procedure. Ultrasound stimulates tissue without incisions. A surgeon can reposition and secure deeper structures and remove excess skin, so the method is selected according to the extent of the concern.

Who May Not Be Suitable for the Procedure

SMAS lifting is not performed during pregnancy, in the presence of active skin damage or inflammation in the treatment area, or with certain systemic conditions. Information about metallic implants in the face, previous surgery, threads, fillers, and other procedures is important. The physician reviews the complete list of restrictions during the consultation, taking into account the device instructions and the patient’s medical history.

The procedure should also not be selected if the patient expects lifting alone to reduce deep static wrinkles, wants a large amount of excess skin removed, or anticipates the outcome of a surgical facelift without surgery. The realistic aim of Ulthera is to improve tissue density and position within the capabilities of a non-invasive method.

How the Procedure Is Performed at Medest

  • The physician assesses the face at rest and in motion, as well as skin quality, the degree of ptosis, and symmetry.
  • After cleansing, the treatment areas are marked and contact gel is applied.
  • Using ultrasound imaging, the specialist identifies tissue depth and selects the appropriate transducers.
  • Ultrasound lines are delivered according to an individual plan; the number of lines depends on the treatment area and anatomy.
  • After treatment, the physician explains which reactions are expected and when the result should be assessed.

Temporary redness, sensitivity, mild swelling, or tenderness to touch may occur after the session. Tingling or tissue sensitivity can occasionally last longer. Increasing pain, marked asymmetry, altered sensation, or other unusual symptoms require prompt contact with the clinic.

How to Choose Between Ulthera and Surgery

The decision begins with three questions: what has changed, what result is required, and how prepared the patient is for recovery. With moderate ptosis, the physician may recommend Ulthera System SMAS lifting. When there is marked excess skin and the patient wants a substantial change in contour, an opinion from a plastic surgeon is required.

At Medest, the physician will perform an in-person assessment, evaluate the degree of age-related change, and explain what result can reasonably be expected from Ulthera in your case. Book a consultation and SMAS lifting treatment in Yerevan to receive a personalised plan that is not based on an arbitrary age threshold.
